Essential Duties and Responsibilities may include the following, depending on the departmental assignment and services provided. Other duties may be assigned.
- Carries out the supported work plan or pre-vocational plan for Program participants by providing community based employment supports and assessments to individuals with disabilities one-on-one, or in small group settings.
- Collects, organizes, and analyzes information about the people supported through records, tests, interviews, and professional sources, to appraise their interests, aptitudes, abilities, and personality characteristics, for the purpose of employment planning
- Maintains regular, effective communication with individuals, their families, agency staff, non-agency staff, business community contacts, and colleagues to support the objectives of their plans.
- Consistently monitors the work performance and progress of people receiving services through regularly scheduled job site visits, face to face discussions and phone contact with job-seekers, employees and their employers.
- Attends and participates in planning meetings for people receiving services to discuss progress and provide collaborative support
- Provides instruction and assistance as needed to individuals in areas including but not limited to workplace expectations, career development, personal hygiene, use of technology, social interaction, organization, stress-management, mobility training, benefits management, and personal finances
- Models appropriate/acceptable social and work skills.
- Compiles and studies occupational, educational, and economic information to aid the individuals we serve in making and carrying out employment objectives.
- Helps ensure appropriate safeguards for individuals are developed and maintained
- Maintains detailed electronic documentation in regard to all services provided in a timely and precise manner. Completes and submits required daily and monthly documentation according to state and federal guidelines for each service type.
- Completes all mandated reporting requirements by Wildwood and regulating agencies within the prescribed time period, including but not limited to the following:
- Habilitation plan service delivery checklists, notes and summaries.
- Pathway to Employment documentation (Action Plan, Discovery Plan, etc.)
- Incident reports
- Documents related to behavioral support plans
- Provides crisis intervention as assigned/needed to individuals to address their challenges at work, within their pre-vocational program, or in their personal life to the degree that those challenges impact their ability to be/remain employed.
- Embraces team approach, attending and participating in all staff meetings and mentoring new staff. Assists with coverage within, and across programs as needed.
- Represents the Employment Services Program and Pre-vocational Services to area employers, co-workers, agencies and the general public to advance the recognition of individuals with disabilities as valued members of the workforce.
- Independently interfaces with community partners. Works and manages time in the community independent of direct oversight, maintaining an established threshold of billable hours.
- Coordinates daily WRI schedules including classroom and community-based activities. Negotiates and manages schedules and daily activities, including making regular adjustments to accommodate the needs of individuals and Wildwood’s business partners
- Assists with the development and implementation of the Work Readiness Curriculum including planning lessons, drafting materials and delivery of lessons.
- Remains current in all agency and program related training.
- Implements emergency procedures and provides CPR and first aid as necessary. All supervisors, managers, and employees are all responsible for helping to create a workplace free of recognized hazards, and for acting safely.
Associates Degree from a regionally accredited college or university in Education, Human Service, or related field, and two years of related experience, or the equivalent combination of education and experience. BA Strongly Preferred. Experience working with people with disabilities is preferred.
Ability to read and interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals. Ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations. Ability to write reports and business correspondence. Ability to effectively present information and respond to questions from business contacts, other agency staff, family members the people we support and the general public.
Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals. Ability to calculate figures and amounts such as discounts, interest, commissions, rate, ratio, proportions, percentages, area, circumference, and volume. Ability to apply concepts of basic algebra and geometry.
Ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists. Ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form.
To perform this job successfully, an individual should have knowledge of word processing software, database software and mobile computing.

While performing the duties of this job, the employee is regularly required to talk or hear. The employee is occasionally required to stand, walk and reach with hands and arms. A valid NYS driver's license and driving record acceptable for agency insurance coverage are required. Daily access to a personal vehicle is required (mileage is reimbursed). Staff may be required to transport individuals in an agency vehicle or their personal vehicle based on program needs.
Must be cleared and maintain acceptable record under the NYS OPWDD mandated criminal background check process
Requires the use of laptop computers (agency-provided) to record activities for billing in fifteen minute increments and requires a high degree of accuracy and attention to detail.
