A Glimpse Into Both of Wildwood's Day Hab Programs

Frequently Asked Questions: Day Hab

Our site based day habilitation programs allow for individuals to participate in various activities in the community and at their site. The individual will have the ability to choose activities of interest to participate in.  Our site based programs are able to pass medication, support with ADL skills,  and can follow behavioral support plans.

Our WOW Day Habilitation program is 100% out in the community. They do not have a site. Staff are not able to pass medication and individuals in this program must have a higher level of independence when in the community. This includes the ability to use public restrooms with no assistance and are able to navigate the community with little to no intervention from staff support.

Site based is 8:30 am-1:45pm (this ranges based upon drop off and pick up times)

WOW program 8:00 am to 2pm (this ranges based upon drop off and pick up times)

Both programs operate on Monday-Friday.

*Please note that we do not have program on the following Holidays: New Years Day, Memorial Day, Juneteenth. July 4th, Labor Day, Thanksgiving and the day after. Christmas Eve and Christmas Day.

Site Based programs work with a Behaviorist, Nurse, and Dietician who are on site at least once per week to support the people attending as well as the team. Our WOW program does not have additional support services during program hours.

Each program Site and WOW provide transportation to and from the program. Everyone must fall into the program catchment area for services. Our catchment area is a 15 mile radius from the program’s physical addresses.  We have programs in East Greenbush, Colonie, and Halfmoon.

All programs work on independence skills which involve participating in volunteer opportunities, art, cooking, and other areas of interest of the individuals within the program. Monthly schedules are utilized by each program.

The program is paid for by the HCBS Waiver via Medicaid. Any additional cost associated with admission costs and personal expenses will be distributed ahead of the event and be shared for families to choose activities of choice.

Each program operates in a small group setting.  On average there is a group sizes of 1 staff to 4 individuals, but can fluctuate based upon the level of independence and need of the individuals within each group.

Each program has a waiting list. To be placed on the waiting list your Care Manager will be required to complete the following form: https://forms.gle/2k7T7ifBYZzsHhUEA and submit the NOD09 service approval to program administration.

If you have Self-Directions services and interested in Day Habilitation services you will need to first get service approval for Day Habilitation from OPWDD via your Care Manager.  Once approved for the service you will need to be placed on a waiting list. Once approved for the service you will need to either build the cost of the program into your existing budget or to move to a Residential only budget, which will move the cost of Day Habilitation service outside of your budget.  An individual budget amendment must be completed for an individual to begin services.

Employment Services

The goal of employment services is to support people as they search for  and attain employment through things like resume writing, interview preparation, application assistance and onsite coaching. Through community-based work assessments, training and internships, employment services help people get ready for and find competitive employment. Employment Services depends on strong partnerships with the business community and it helps employers to understand the value of hiring people with disabilities and  realize the full benefit of inclusive employment.
