Job Title: Community Habilitation Specialist
Summary: Facilitates the growth and community interaction of an individual with neurological impairments, learning disabilities, or autism. Provides a variety of services necessary to enable the individual to live successfully in the community while providing relief to individuals who provide primary care and support.
• Works one-on-one in the consumers home and community to address a variety of life skills to include social, recreational and other activities of daily living.
• Follows the goals and/or activities specified in the individual's "Individualized Service Plan" and Community Habilitation Plan.
• In cooperation with the individual and family, the support staff helps to identify skills and competencies to be developed and devises plans to achieve those skills and competencies.
• Assists consumers to identify social, recreation and other community resources the individual may wish to access and devise plans to achieve that access.
• Measures the likelihood of social isolation and develops plans to build social and communal relationships, which prevent or minimize that isolation.
• Negotiates schedules and daily activities.
• Regularly maintains and submits service notes with documentation of the hours worked and the service provided.
• Maintains and submits travel vouchers and reimbursement requests.
• Follows any agreed upon action plan for activities with the individual, including a behavior plan.
• Employs knowledge of emergency procedures as outlined by Wildwood.
• Enhances and promotes social interaction and community participation.
• Presents as a positive role model.
• Works those hours authorized by the Individualized Service Plan (ISP).
• Submits to and reviews reports and problems with the Assistant Director for Community Habilitation.
• Provides transportation for the consumer to and from community activities.
Education/Experience:
High school diploma or general education degree (GED); and one to three months related experience and/or training; or equivalent combination of education and experience.
Additional Requirements:
• Daily access to an automobile is required. A valid New York State Driver's License and ability to drive and driving record acceptable for agency insurance coverage are required.
• CPR and First Aid certification or eligibility for certification.
• Applicants hired after April 1, 2005 must be cleared and maintain acceptable record under the NYS OPWDD mandated criminal background check process.
Albany County Opening:
RELIEF OPENING- We are looking for an experienced direct support professional to provide relief for other team staff working one to one with a 21 year old male who lives with his mother in Albany. The hours are Monday – Friday between 8am and 6 pm. Hours would vary depending upon consumer needs. The individual wants support to maintain and expand his current community inclusion activities, including swimming, taking swim lessons at the local Y, participating on the Special Olympics Track Team, volunteering, walking, food shopping, and continuing to develop effective communication skills.
Malta Opening for EXPERIENCED professional:
We are looking for an experienced Community Habilitation Specialist to work 6 hours each week (one weekday afternoon/evening and one Saturday) with a very active 7 year old boy with a Developmental Disability who lives with his parents in Malta. He enjoys going to the local library, playing on his trampoline, going for walks, bowling, and playing both active and sedentary games. He is looking for support to develop safety awareness skills in both his home and community; along with age appropriate social, play and leisure skills.
Albany County Openings:
ALBANY- We are looking for 2 experienced direct support professionals to work one to one with a 28 year old woman who lives with her mother in Albany. She wants support Monday-Friday, from approximately 4-7 pm. She needs support to continue to maintain and increase her abilities with banking, food shopping, and being responsible for many aspects of running the household. She wants support in areas of clear verbal communication and many areas of personal safety. She is very social and enjoys connecting with others and being an active member of her community.
ALBANY- We are looking for an experienced direct support professional to work one to one with a 21 year old man who lives with his mother in Albany. We are looking for someone to join an existing team for approximately 30 hours per week, Monday-Friday, 12-6 pm . This position comes with prorated benefits. He wants support to maintain and expand his current community inclusion activities, including swimming , taking swim lessons at the local Y, participating on the Special Olympics Track Team, volunteering, walking, food shopping, and continuing to develop effective communication skills.
ALBANY- A young adult man living in Albany is looking for a Community Habilitation Specialist to take him out into the community on either a Monday or Friday evening. He would like to address skills such as communicating clearly, safety skills, healthy lifestyle, and accessing the community for a variety of learning and recreational experiences. He enjoys dining, dancing, swimming, bowling, and other sports.
ALBANY- A 15 year old boy living in Albany with his mother is looking for community hab 1-2x per week to access the community for activities such as sports, museums, movies, dining, and the playground. He would like help with safety and being independent. Reminders/assistance for keeping his space at home tidy. Would like to work on basic cooking skills. Assistance with functional reading skills
LATHAM-A young adult woman would like to access the community a couple times per week for physical activity. She enjoys biking, walking, hiking, working out at the Y, and occasional shopping. She is working on safety skills, daily living skills, and coping/decision making. She is available Wed, Fri, or weekend hours.
ALBANY- A young adult male who lives in Albany with his parents would like to access the community on Saturdays after 2:00 or on Sundays. He enjoys staying active in the community through accessing local parks for walking, and the Y. He also enjoys movies and the library. He would like to work on independence and socialize in his community.
COLONIE- An 11 year old boy living with his mother and preparing to enter the junior high school is looking for comm. hab after school in his home. His main focus is working on safety
Saratoga County Openings:
SARATOGA- A 16 year old Wildwood student is looking for an experienced Community Habilitation Specialist to work with him in Saratoga Springs. He is looking for someone to work with him on Saturdays for four hours to support him at sports activities such as SAFE, Challenger, Special Olympics; going to the library; attending local high school sports events; and to become a regular in his community. He likes reading, walking, sports and helping others.
SARATOGA- An adult male is looking to access the community once per week with day-of-the-week flexibility, during the daytime for a variety of activities such as cooking, volunteering, and fitness and recreation opportunities. He would like support with increasing independence and self-advocacy.
CLIFTON PARK- A 30 year old male, living in Clifton Park with his large family, is looking to access the community 1-2x per week for social, recreational, and healthy living activities. He enjoys sports, country music, bowling, working out at the gym, movies, concerts, and dining. Needs education/encouragement regarding healthy food choices, fitness, healthy grocery shopping/cooking.
CLIFTON PARK- A 18 year old male living in Clifton Park with his mother is looking to access the community once per week to work on health, fitness, recreation, safety, and independence. He enjoys biking, swimming, bowling, and dining.
MECHANICVILLE- A male in his 30’s living with his parents is looking for a community habilitation worker to access the community on the weekends. He would like assistance with appropriate and effective communication and safety in his community. He enjoys being a part of his community and getting to know people he sees regularly.
CLIFTON PARK- A 16 year old Shenendehowa student would like support approximately 10 hours per week in his community of Clifton Park. He would like support in areas of hygiene and appearance, so he can access the local Y, library, hiking paths, malls. He wants support with improving his coping skills regarding areas of peer interactions and being bullied. He wants to shop & learn to prepare simple meals and learn to do his laundry. He enjoys reptiles & sharks and has a vocational interest in Oceanography.
Rensselaer County Opening:
CASTLETON- A 13 year old girl living with her parents and older brother, is looking for an additional CHS to work with her on communication, safety, and daily living skills such as waiting, eating, dressing, shopping, and hygiene tasks. This CHS would need to be very creative, musical, and experienced in working with people with autism.
